The vendor later sent a batch where the force measured 3.5 N across 200 units. Normal tolerance in the industry might be ± 0.5 N, but our internal standard was tighter. We rejected the whole batch, and the vendor had to redo it at their cost. That delay cost us a $22,000 launch postponement.

For manual resuscitators, specify the volume per squeeze (e.g., 450–600 mL for adult), pressure relief valve setting, and bag material durometer. For medical devices, hidden costs can be brutal. Setup fees for custom tooling (e.g., for a unique surgical stapler cartridge), sterilization validation, packaging design, shipping with temperature logging, and warranty terms that shift liability. One supplier quoted a “great” per-unit price for patient monitors, but their contract had a $1,200 annual software license fee per unit and a $0.10 per-use data export charge. Step 3: Verify Certifications and Quality Records—Don’t Take Their Word

This seems obvious, but I’ve seen buyers accept a scanned ISO 13485 certificate that expired two years ago. It happens way more often than you’d expect. For ICU devices, you need proof of:

  • ISO 13485:2016 certification (current, from an accredited registrar)
  • FDA 510(k) clearance or PMA for devices sold in the U.S.
  • CE marking (MDR) for Europe
  • FDA establishment registration for the manufacturing site

But don’t stop at certificates. Ask for the latest audit report (redacted as needed) or a quality scorecard from a previous customer. If they balk, that’s a serious red flag. In 2023, I almost approved a supplier for manual resuscitators until I requested their CAPA history. Turned out they had four open corrective actions for balloon leakage—something that could be fatal in an ICU setting. We walked away.

Request samples from each category. Run a blind test with your team: same product from two different batches, or from two different product lines. Step 5: Order a Small Batch and Do a Full Acceptance Test (The Step Most People Skip)

This is the one that catches most buyers off guard. They negotiate everything, sign the contract, and then place a huge order. Then a problem surfaces three months later when 8,000 units are already in storage.

Instead, order a small batch—say 10–20 units of each device type. Then run a full acceptance test according to your spec sheet. Measure, inspect, stress-test. For a surgical stapler, fire it 1,000 times into test foam and check for misfires. For a manual resuscitator, check bag recovery time after full compression across 500 cycles. For a pressure mapping system, compare output readings against a calibrated gold standard.

I’ve never fully understood why procurement teams skip this. My best guess is they’re worried about delaying the project. But the cost of a small trial is nothing compared to a full rejection. In our 2020 fiscal year, we didn’t have a formal first-article inspection process. That cost us when a batch of patient monitors arrived with the wrong alarm thresholds. Redoing 200 units took six weeks and lost us a hospital contract.

So glad I now insist on this step. Almost let the supplier talk me into “we have a 99% pass rate on first submissions” without hard data. Would have been a bullet dodged, but now it’s a standard contract clause.

Common Mistakes to Avoid

  • Only asking about price first. You’ll get a low number and later learn about add-ons. Always start with specs, then request total cost breakdown.
  • Trusting certificates without verification. An ISO 13485 certificate means nothing if it’s expired or not scope-specific. Call the registrar if needed.
  • Assuming “one-stop shop” means equal quality. Verify each product line separately.
  • Neglecting the small batch test. This is where real problems surface. Insist on it in the contract.
